Once your forms are returned via email;

• We will reply to you:

  • Confirmation of booked dates

  • A populated service agreement

  • A copy of QTR cancellation policy

  • Client Profile Pages 3-6

  • Consent for personal images

  • Government SA / SA Health Medication Authority (if relevant)
